To be eligible to participate, practice, or play on a sport team at FHS, you must turn in all athletic paperwork each year. Remember, athletic paperwork does not "transfer" or "carry-over" from the previous school year. Come into the Attendance Office to receive a complete athletic packet. If you have questions regarding the forms, insurance, athletic fees, or athletic scholarships please contact our Athletic Secretary, Ms. JJ Wilkerson firstname.lastname@example.org (206-252-6282). Once your packet is completed, turn in all paperwork to the Attendance Office.
In addition to the athletic packet, each individual sport requires a separate "Sport Risk/Injury Consent" (Consent) form to be reviewed, signed and returned with an original signature by parents and student athlete. (For example, if an athlete participates in Cross Country, Basketball and Soccer, a consent form for each sport is required.) All of the Sports Risk/Injury Consent forms are electronically available below, as well as physically available in the Attendance Office.
Click on the sport in the list below to download the consent form for that sport.
* Please note 2 forms are needed for these programs.
