Spring 2024 Registration
Spring sports will begin on February 26. Please register as soon as possible. The online registration process must be complete and all athletes cleared in order to be eligible and participate in practices.

How to Register
Franklin HS uses FinalForms for online athletic registration. Data is saved season-to-season and year-to-year, so you will never need to enter the same information twice. Data is pre-populated in the form fields wherever possible.
You may log in any time to update information such as emergency medical contacts. You will be required to sign your forms once per year and any time you make an update.
Parents/Guardians must start the FinalForms registration process before student athletes can log in to their account and set their password. Both student and parent/guardian are required to complete and sign all forms before students can tryout or start practicing.
You will need the following information for your student athlete to allow for accurate completion of the Online FinalForm Registration Process
- Students Personal Email Address
- Basic Medical History & Health Information
- Insurance Company & Policy Number
- Doctor & Dentist Contact Information
- Hospital Preference
- A Completed Physical
- Any extra WIAA required forms (Running Start, New to SPS, Out of District, etc.)

Instructions for FinalForms Registration
- Go to seattleschools-wa.finalforms.com and under Parent select *New Account.
- Follow the Registration Steps, including enabling translation if English is NOT your first language. If you need assistance creating your account, please contact FinalForms Technical Support at support@finalforms.com.
- Check your email for a message from FinalForms. You will receive an email within about 5 minutes prompting you to Confirm your account. If you do not receive an email, check your spam folder. If you still do not see the FinalForms email, please email support@finalforms.com.
- Set up your FinalForms new password and select Confirm Account.
- Select Register Student
- Type Students basic contact and demographics information and select Create Student.
Note: Use student’s Personal Email not the @seattleschools.org email address. - Check the box next to any sports your student plans to participate and select Update after making your selection.
Note: A selection can be changed any time UNTIL the registration deadline. After the deadline has passed please contact the athletics secretary to add a sport. - Your student will now need to go to their email and follow the instructions in their confirmation email to create their password. **Important*** An email will automatically be sent to the email address that YOU provided for your student, which will prompt your student to sign their portion of the athletics forms.
- Students and Guardians will need to Complete each form and Sign Their Full Name on the Parent or Student Signature field at the bottom of the page. After signing, select Submit Form and move on to the next form.
- Incomplete forms will appear in Red along the side of the screen. You can switch between forms by clicking on the names of the forms.
Note: A Green Check means the form is signed by both guardian and students, an Orange Student means it is signed by the guardian but not the student, and a Red Guardian means the form needs to be signed by guardian (and possibly the student too). - When all available forms are complete, you will see a Forms Finished message.
Note: This message means that all forms that have populated to your account are completed. This message is not the same as confirmation of clearance or completion of forms from the athletics office.
When new forms are added to your account, follow the steps below for completing incomplete forms. If you have not chosen a sport you will see this message prematurely.
Registering Additional Students
- Go to seattleschools-wa.finalforms.com and under Parent select Login. Enter the email address you used to create the account, and your FinalForms password.
- On your My Students Dashboard select Add Student. To get back to this page, select the Seattle Public Schools logo in the upper left-hand corner Or the My Students logo in the upper right-hand corner.
- Repeated steps 6 through 11 from the Step by Step Instructions for FinalForms Registration above.
Completing Additional or Incomplete Forms
- Go to seattleschools-wa.finalforms.com and under Parent select Login. Enter the email you used to create the account, and your password.
- On your My Students Dashboard click Incomplete Forms. To get back to this page, select the Seattle Public Schools logo in the upper left-hand corner Or the My Students logo in the upper right-hand corner.
- On the next page it will ask you to verify the sports your student athlete is planning on participating in. If there are no changes, click the No Changes, proceed to forms link at the bottom of the page.
- On the next page you can see which forms will need attention on the left-hand side of the window. You can switch between forms by selecting the names of the forms.

Updating Forms
- Go to seattleschools-wa.finalforms.com and under Parent select Login. Enter the email you used to create the account, and your password.
- On your My Students Dashboard select, Update Forms.
- On the next page it will ask you to verify the sports your student athlete is planning on participating in. If there are no changes, select the No Changes, proceed to forms link at the bottom of the page.
- On the next page you can navigate to the forms you want to update and switch between forms by selecting the names of the forms along the left-hand side of the window.
Note: A Green Check means the form is signed by both guardian and students, an Orange Student means it is signed by the guardian but not the student, and a Red Guardian means the form needs to be signed by guardian (and possibly the student too). - After updating each form, select Submit Form.
- Guardians Register online via FinalForms. Create an account, add your student with their personal email
- Both student and guardian are required to complete and sign all forms, please use your students’ personal email address as FinalForms gets rejected by the school email accounts. Students need to check their email AFTER guardians have registered them on FinalsForms.
- Get a Physical for your Student Athlete(s).
Notes:
Physicals are valid for two years from the Exam Date . Your student will need an updated physical to participate in the open season and/or before participating in sports during the coming year.
If you need a copy of the form for the doctor to fill out, download the physical form. - Turn in your Physical once the physical is completed. There are three ways that you can turn in the Physical, all three work great.
- Upload a scan or a photo of the physical directly through final forms. Log in and then select the “Student’s Documents” folder button (figure 1) in the Actions section of your student’s listing
- E-mail a picture or scanned copy of the physical directly Athletics Secretary, JJ Wilkerson at jmwilkerson@seattleschools.org
- Drop a paper copy of the form in in the main office
- Pay ASB fee on SchoolPay via the Source or in person at the Fiscal Office.
Note: Final Athletic Clearance from the Franklin HS Athletic Office is required prior to participating in any WIAA sports.
Up-to-date Physicals must be good from and be current from the first day of tryouts/practices until the day after the State Playoffs. Late paperwork may limit participation for tryouts and practices.
Physical Expiration Dates are able to be viewed and are located on students Source Accounts and though FinalForms within a week of being turned in to the Athletics Secretary (during the school year). Physicals are valid for 24 months from the Exam Date listed by the doctor on the Physical Form. Please make sure your doctor includes the exam date, and a readable doctor’s stamp with signature. Note: Middle school physicals do not automatically transfer to High School, in order to ensure student-athlete info is current and correct physicals for incoming freshman will need to be resubmitted to the FHS Athletics Secretary.

Return to Play Form – Required if you are seeking a physician to give medical clearance to return to athletic participation after a sickness or injury. It must be returned to Franklin’s Athletic Trainer or the Athletics Office.

Students Not Enrolled Full-time at Franklin HS “Within District”
Students are required to complete a WIAA contract if they do not attend classes at Franklin HS full-time. Depending on your enrollment status please follow the instructions below.
Running Start
If you are enrolled in Running Start, please complete the WIAA Running Start Contract on the WIAA website or download it from the WIAA HS Athletics Eligibility form in FinalForms. To find the form on the WIAA website, select Expand below Student Eligibility and Related Issues and then scroll down to find the WIAA Running Start Contract . Download a copy of your Running Start class schedule. Upload the Running Start class schedule and contract to your FinalForms account.
Off-Campus Classes/Alternative Schools
If you are enrolled in any off-campus classes, please complete the Alternative Education School Contract on the WIAA website. To find it, select Expand below Student Eligibility and Related Issues and then scroll down to find the Alternative Education School Contract .
Download a copy of your class schedule and upload the schedule and the Alternative Education School Contract to your FinalForms account.
IMPORTANT: You must complete the two steps below prior to uploading documents to your FinalForms account.
- Confirm your neighborhood school by using the ” Address Look Up Tool“.
- Pay ASB fee and/or any additional team fees or fines on SchoolPay via the Source or in person at the Fiscal Office and upload the SchoolPay receipt to your FinalForms account.
Grades
Grade checks are run throughout each sports season. Please upload a copy of previous semester and most recent quarter grades to your FinalForms account. Failure to provide grades may make you ineligible for competition. If you attend any other school for ANY classes, you must provide your grades from that school. Student Athletes must have a 2.0 GPA in 5/6 Classes . All students must have and maintain a 2.0 GPA or greater, in 5/6 classes to participate in WIAA sports. Refer to the SPS Student Athletic Handbook for more details.

Not Enrolled Full-time at Franklin HS “Out of District” Students
Important: All “Out of District” athletes must also follow the instructions below.
- Confirm your neighborhood school by using the ” Address Look Up Tool“.
- Guardian/student completes SPS Online Enrollment. If you have any difficulty uploading documents, or have not received a confirmation email from Seattle Public Schools within two days of completing athletics enrollment, please contact Susana Flores at SEFlores@seattleschools.org. Once enrolled, please contact the FHS Athletic Office
- Guardian created FinalForms Account. Refer to the instructions in FinalForms Registration Instructions located in the tab above.
- Get and Upload current physical forms to your FinalForms account. Refer to the instructions under the Physical tab above.
- Pay ASB fee on SchoolPay via the Source or in person at the Fiscal Office and upload the SchoolPay receipt to your FinalForms account.

Off-Campus Classes, Private Schools, & Home School

Download a copy of your class schedule for your school. Upload the class schedule and Alternative Education School Contract to your FinalForms account.
Home School
If you are home schooled, please complete the Home Based Education School Contract on the WIAA website. To find it, select the Expand below Student Eligibility and Related Issues and then scroll down to find the Home-Based Education School Contract .
Upload your completed “Home Based Education School Contract” to your FinalForms account.
